Where the money goes

Most practices do not have a strategy problem. They have a capacity problem.

Every unworked claim, expired credential and unbenchmarked supply order quietly compounds. These are the three places we find it first.

Front office

Phone calls that never become appointments

  • New-patient enquiries going to voicemail at lunch and after 5pm
  • Referrals faxed in and never scheduled
  • No-shows nobody calls back to refill
  • Front desk staff doing credentialing instead of patients
15–30%of inbound new-patient calls never convert to a booked visit in practices without dedicated intake coverage.

Revenue cycle

Money you earned, sitting in a queue

  • Claims held for missing information until they age out
  • Denials worked weeks after the appeal window closes
  • Underpayments posted as contractual adjustments
  • A/R over 120 days that nobody owns
5–10%of net revenue is typically lost to preventable denials, underpayments and timely-filing write-offs.

Overhead

Costs growing faster than collections

  • Supply spend creeping up with no benchmark behind it
  • Locum and agency coverage at premium rates
  • A licence, attestation or revalidation quietly expiring
  • Turnover that resets the whole training cycle
8–15%of operating expense is usually recoverable through contract alignment, benchmarking and process discipline.

Services

Ten services. One operator.

Start with the one that hurts most. Every service is staffed, priced and reported independently — and they are designed to sit together without a second onboarding.

AI agents

Or let agents do it — with humans on the exceptions.

Thirteen specialised agents answer the phone, draft the note, file the fax, chase the prior auth and post the payment, inside the EMR you already use. Every action is logged, reviewable and reversible, and anything below threshold lands with a certified human.

Why practices stay

Not a vendor. An operator.

We are measured on the same numbers you are: collections, days in A/R, denials, staffing cost and compliance exposure.

We work in your systems

No forced migration, no new platform, no per-seat licence. Your team keeps the tools they already know, and we work inside them under your credentials.

One operator, not six vendors

Billing, coding, credentialing, audits, HR, supplies and technology under one agreement, one account manager and one reporting pack.

Specialty-matched people

Your account is staffed by coders and billers who work your specialty, not a generalist queue that needs a cheat sheet for every claim.

Metrics you can audit

Clean-claim rate, days in A/R, aging, denial reasons, credentialing status, supply spend. Reported monthly, in writing, with the underlying detail available.

Vendor-neutral by design

We take no commissions from EHR vendors, distributors, device makers or buyers. Our advice is the product — not the referral.

Compliance built in

A BAA before data moves, HIPAA-aligned workflows, a documented audit trail on every touch, and audit-ready workpapers on request.

How an engagement starts

Assess, plan, transition, report.

No big-bang cutover, no six-month discovery project. We start by measuring, and we show you the measurement.

01

Assessment

A structured review of your revenue cycle, coding, credentials, staffing, supply spend and compliance posture. We quantify the gap before we propose anything.

Week 1–2Fixed fee or free
02

Plan & pricing

One written plan naming the services, the sequence, the staffing and the price — with the assumptions stated so you can argue with them.

Week 2–3
03

Transition

We move in alongside your team, work their queues, learn your payer quirks and take over volume gradually. No big-bang cutover.

Week 3–6
04

Operate & report

Monthly review of the metrics that matter, a written action list, and quarterly re-benchmarking against your own baseline.

Ongoing

The benchmarks we manage to

These are the industry-standard targets we report against every month. Your baseline is measured in the first two weeks, and every subsequent report shows the gap closing — or explains why it is not.

Who we serve

Solo to PE-backed groups.

Solo & small practices

One to three providers who need a billing office, a credentialing team and an HR function without hiring all three.

We usually start with billing and credentialing, then add supplies benchmarking once the revenue is stable.

Growing multi-provider groups

Four to thirty providers across one or more sites, where the operations layer has to scale faster than headcount.

Billing, coding, credentialing and HR run as one operation with consolidated reporting across locations.

New practices & new locations

Physicians leaving a group, partners splitting, or an existing practice opening a second site.

We run the launch workstreams and get you billable on day one rather than day ninety.

Specialty practices

Cardiology, orthopaedics, dermatology, OB/GYN, behavioural health, GI, urology, ophthalmology, oncology, sleep and more.

Coders are matched to your case mix; auditors and benchmarks are specialty-specific.

Private equity & MSO-backed groups

Platforms that need standardised revenue cycle, clean diligence data and reporting a board can read.

We standardise across acquired practices and produce the metrics buyers and lenders expect.

Practices preparing to sell

Owners planning an exit in 12 to 24 months who want a higher multiple and a cleaner diligence file.

We clean up coding, credentialing, contracts and financial reporting so the diligence file tells your story.

How we work with each type of practice

FAQ

The questions we get asked first.

What size practice do you work with?
From solo practitioners to groups of about 120 providers. Below three providers, a targeted engagement on billing or credentialing usually makes more sense than the full stack. Above 120, we scope a dedicated team rather than adding your account to an existing pod.
Do you replace our staff?
Rarely, and never by surprise. Most practices redeploy their billing and administrative staff onto patient-facing and clinical-support work while we absorb payer calls, appeals, posting and compliance. Where a role genuinely changes, we help you plan the transition.
How is your pricing structured?
Four models: percentage of net collections, flat monthly fee per provider, fixed-fee project, or contingency for credentialing and practice sales. We recommend the model that matches the work rather than the one that suits us.
What systems do you work in?
Athena, eClinicalWorks, NextGen, Epic, Tebra, AdvancedMD, ModMed, Elation, DrChrono, Kareo and about 30 others, plus most clearinghouses. If you are evaluating a new system we stay vendor-neutral and take no commissions.
How do you handle patient data?
A signed BAA before any data moves, least-privilege access under named accounts, encryption in transit and at rest, MFA, and a documented audit trail on every action. We are HIPAA-aligned and we will provide our security documentation during diligence.
What does onboarding look like?
Two weeks of assessment and planning, then a staged transition over four to six weeks. You get a named account manager, a written plan with dates, and a weekly call until the metrics are where they should be.
Can we start with just one service?
Yes. Credentialing, auditing and supply benchmarking are the most common entry points because they are self-contained and produce a visible result quickly. Everything else is designed so a second service can be added without re-onboarding.
Who owns our data and our contracts?
You do — always. We work under your tax ID, in your systems. On exit you receive every work queue, policy document, report and workpaper in a standard format, and we will support a transition to another vendor or back in-house.
