Effective date: 1 January 2025 · Last reviewed: 1 January 2025
Overview
Stat Physician LLC ("we", "us") provides business, administrative and consulting services to physician practices. This notice explains what information we collect through this website and in the course of an engagement, how we use it, and the choices you have.
We are not a covered entity. When we handle protected health information on behalf of a client practice, we do so as a business associate under a signed Business Associate Agreement (BAA).
Information we collect
Information you give us: your name, practice name, email address, telephone number, specialty, systems in use and the content of your enquiry when you submit the contact form or email us.
Information collected automatically: standard web server logs including IP address, browser type, referring page and pages viewed. We use this for security and to understand which pages are useful.
Cookies: this website does not set advertising or tracking cookies. Any cookie set is strictly necessary for the site to function.
How we use information
To respond to your enquiry and provide the services you ask for.
To operate, secure and improve this website.
To meet legal, regulatory and contractual obligations, including audit and record-keeping requirements.
We do not sell, rent or trade personal information. We do not use your information to train third-party advertising models.
Protected health information
We do not ask for and you should not send protected health information through this website, by email or by text message.
Before any protected health information is exchanged, we execute a Business Associate Agreement and provide a secure channel for the data.
Where we act as a business associate, we use and disclose protected health information only as permitted by the BAA and by HIPAA, and we apply administrative, physical and technical safeguards to it.
We maintain workforce training, access controls, audit logging, encryption in transit and at rest, and a documented incident response procedure.
When we share information
With service providers who help us operate (for example hosting, email and accounting) under contracts that restrict their use of the information.
With your consent, or at your direction, for example when we contact a payer or hospital on your behalf.
Where required by law, regulation, subpoena or court order.
Your choices and rights
You may ask us what personal information we hold about you, request a copy, ask us to correct it, or ask us to delete it where we are not required to retain it.
You may ask us to stop sending you commercial email at any time; every message includes an unsubscribe link.
Depending on your state of residence, additional rights may apply, including rights under California, Colorado, Virginia and Connecticut privacy statutes.
To exercise any of these rights, email info@statphysician.com.
Retention and security
We retain enquiry records for as long as needed to respond and to maintain a business record, and client engagement records for the period required by law and by our professional obligations.
We protect information with access controls, multi-factor authentication, encryption, logging and periodic review. No system is perfectly secure, but we treat this as an operational discipline rather than a statement.
HIPAA notice
This website is not a covered entity's notice of privacy practices, and it does not describe how your physician uses or discloses your health information. For that, contact your physician's office directly.
If you are a patient of one of our client practices and have a question about your records, please contact that practice. We cannot respond to patient record requests directly.
Our HIPAA-aligned safeguards include: a signed BAA before any PHI is exchanged; least-privilege access under named individual accounts; encryption of PHI in transit and at rest; audit logging of access to PHI; workforce HIPAA training with annual refresh; and a documented breach notification procedure.
